How to Make a Referral to Revolution Case Management

We welcome referrals from solicitors, insurers, health and social care professionals, and families. Our aim is to make the referral process as straightforward as possible.

Please read the following guidance before completing the referral form.

Step 1: Who Can Refer?

Referrals can be made by:

  • Case managers or rehabilitation teams
  • Solicitors and deputies
  • Insurers
  • NHS or private clinicians
  • Family members or legal guardians (with consent)
We accept referrals for both children and adults with complex needs, including acquired brain injury, spinal cord injury, amputations, and neurological or orthopaedic conditions.

Step 2: Information Required

To help us understand the client’s needs and respond efficiently, please provide the following:
  • Referrer details: Name, organisation, role, contact information
  • Client information: Full name, date of birth, contact details, legal representative (if applicable)
  • Nature of injury or diagnosis: Brief summary and date of injury
  • Current situation: Hospital/discharge status, current support, any risks or concerns
  • Reason for referral: Goals of case management, any specific services required
  • Funding information: Insurer, solicitor, or other funding arrangements
  • Supporting documents (optional but helpful):

    • Medical reports
    • Therapy assessments
    • Risk assessments
    • Care plans

What Happens Next?

  4. Service Proposal: If appropriate, we will develop a tailored case management proposal, including estimated hours and costs.
